This is SQL Server 2014 on Windows Server 2012, already in production use. 2014 as you know does not support Basic Availability Groups. Can I configure Always On 2014 with only 2 SQL Servers, a primary and a secondary?

If by 2 SQL Servers - you mean 2 Windows servers with Windows Cluster installed, then yes. The requirement is to have a windows cluster - and you can create a windows cluster with 2 nodes. In this configuration you would have either a disk or file witness for quorum.
Thanks, jeffw8713. In my case, we are using a File Share Witness. That is failing and may be due to a permissions issue. More info later.
Make sure the cluster has access to the FSW.
