Unpivot with Column Name

I have this set of data:

How do i transpose or unpivot then to this?

I tried this

UNPIVOT

) As

But is not working fiine. Thanks alot

any other column to determine the sequence or ordering of the record in the table ?

Also, you will only have 2 rows in that table ?
